(A) Address Proof :

1. Valid PassPort
2. Election lD
3. Life insurance Policy + Ration Card +Telephone Bill
4. Post Paid Mobile Bill+ Ration Card + Maintenance Bill
5. lf the above documents are not available then electricity bill plus telephone bill plus ration card along with letter. from the chairman/secretary of the co-operative housing society where the applicant ‘resides may be accepted as address proof.
6. Registered leave and licence agreement only, along with intimation copy of the local police station where the applicant re-sides, supported with proof of permanent address of residence, and letter of employer stating the confirmation of residential address proof submitted by the applicant.

(B) Age Proof :

1. School leaving certificate
2. Birth certificate
3. Valid Passport

(C) Passport size 5 Photo Graphs
